Behaviour Incident Reporting — What To Do

Follow the steps below from the moment an incident happens

A behaviour incident occurs

Staff

Is it minor / routine, or an ABCH / Critical Incident?

If it's minor / routine

Log it directly in the portal — no further steps needed.

If it's an ABCH / Critical Incident, continue to Step 3 below.

Choose how to complete the ABCH report

or, not preferred currently Enter directly in Behaviour Analysis tool

Used the C.I. Bot? Format the report

Staff · skip this step if you used the Word template instead

Paste the completed text in, then save and edit as needed.

Add the report to Teams

Staff · tag @Manager and @TC

Don't forget: log it in the Behaviour Analysis tool too

Staff · via Historical Entry, in the Analysis section

This is separate from Teams and is the step most often missed.

That's your part done. From here, Manager, TC and Admin take over — the steps below are just so you know what happens next.

A few things to keep in mind

The Behaviour Analysis tool isn't the preferred way to complete a report right now — use the C.I. Bot or the Word template instead.

Whichever one you use, you still need to separately log it in the Behaviour Analysis tool via Historical Entry. It's easy to forget because it happens outside Teams.
